Vitiligo is a commonly acquired pigmentary disease characterized by the progressive destruction of melanocytes. The role of adipose tissue in vitiligo remains unclear. To investigate the role of adipose tissue in the pathogenesis of vitiligo, 50 patients newly diagnosed with vitiligo and 40 age- and sex-matched healthy controls were enrolled, and fat mass and distribution using dual-energy X-ray absorptiometry (DXA) and serum adipokine levels using enzyme-linked immunosorbent assay were assessed. The trunk and whole-body fat mass and the trunk/limb fat mass ratio were significantly higher in patients with vitiligo. Higher leptin and resistin levels and lower adiponectin levels were observed. Positive correlation of the trunk fat mass index with vitiligo area scoring index scores (r = 0.38, p = 0.0071) was found. Additionally, plasma adiponectin levels were negatively correlated with vitiligo disease activity scores (r = –0.307, p &lt; 0.05). Receiver operating characteristic curve analysis showed that the percentage of limb lean tissue mass, trunk/limb fat mass ratio, and serum adipokine levels achieved a high area under the curve score for distinguishing patients with vitiligo from healthy controls. In conclusion, the incidence of central obesity and adipokine dysregulation was higher in patients with vitiligo. The potential role of adipose tissue in the pathogenesis of vitiligo should be emphasized.

Exploring the Role of Adipose Tissue Dysregulation in Vitiligo Pathogenesis: A Body Composition Analysis

To explore the effects of transcranial direct current stimulation combined with cognitive training on executive function and activities of daily living performance among stroke patients.A total of 50 subjects were enrolled and randomly allocated into 2 groups of 25 each. The real-transcranial direct current stimulation group was simultaneously subjected to transcranial direct current stimulation and cognitive training, while the sham-transcranial direct current stimulation group was simultaneously subjected to sham transcranial direct current stimulation and cognitive training. At baseline, and after treatment, each subject was assessed with the Wisconsin Card Sorting Test (WCST), Stroop Color-Word Test (SCWT), Digital Symbol Test (DST), Mini-mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA) and Activities of Daily Living Scale (ADLs).After treatment, the gains in most indices of WCST, SCWT, DST, MMSE, MoCA and ADLs in the real-transcranial direct current stimulation group were significantly higher than those in the sham-transcranial direct current stimulation group (p<0.05). Nonetheless, no significant differences were noted in the gains in SCWT (including only Part A time and error, and Part B time) and activities of daily living (including only basic activities of daily living) between the 2 groups (p>>0.05).Transcranial direct current stimulation combined with cognitive training was found to significantly enhance executive function and instrumental activities of daily living performance among stroke patients.

Effect of combined use of transcranial direct current stimulation and cognitive training on executive function and activities of daily living after stroke

The aim of this study was to determine differences in the functional properties of the stratum corneum of children and adults, focusing on the influence of approaching puberty. Biophysical measurements were made of the stratum corneum of 32 healthy Japanese children aged 10-14 years and their mothers in summer and the following winter. The children showed significantly lower skin surface hydration. Stratum corneum barrier function, evaluated in terms of trans-epidermal water loss, was poorer on the forearm in the children than in the adults regardless of season. By contrast, the stratum corneum barrier of the cheek, which was better in the children, tended to become poorer when the children reached puberty. Although the immaturity of the cornified envelopes of the superficial corneocytes, which ratio increased significantly in winter, was not different from that of adults, the corneocytes were significantly smaller in the children, suggesting a more rapid turnover of the stratum corneum. The amount of skin surface lipid, which was measured only on the cheek, remained low until 13 years of age, but at 14 years of age it increased remarkably, approaching adult levels. We conclude that, until puberty, most functional characteristics of the skin of children remain distinct from those of adults.

Functional Characteristics of the Skin Surface of Children Approaching Puberty: Age and Seasonal Influences

This study aimed to identify factors that assessors considered important in decision-making regarding suitability for inpatient rehabilitation after acute severe stroke.Multi-site prospective observational cohort study.Consecutive acute, severe stroke patients and their assessors for inpatient rehabilitation.Rehabilitation assessors completed a questionnaire, rating the importance (10 point visual analogue scale) and direction (positive, negative or neutral) of 15 patient related and 2 organisational items potentially affecting their decision regarding patients' acceptance to rehabilitation.Of the 75 patients referred to rehabilitation and included in this study 61 (81.3%) were accepted for inpatient rehabilitation. The items considered to be most important in the decision to accept the patient for rehabilitation were pre-morbid cognition, pre-morbid mobility and pre-morbid communication. For those not accepted the most important items were current mobility, social support and current cognition. Factor analysis revealed 3 underlying factors, interpreted as post-stroke status, pre-morbid status, and social attributes, accounting for 61.8% of the total variance. All were independently associated with acceptance for rehabilitation (p < 0.05).This study highlights the importance of pre-morbid function and social factors in addition to post-stroke function in the decision making process for acceptance to rehabilitation following severe stroke. Future models for selection for rehabilitation should consider inclusion of these factors.

Porokeratosis is a heterogeneous group of skin disorders. Five clinical variants have been described: classic porokeratosis of Mibelli, disseminated superficial actinic porokeratosis, linear porokeratosis, punctate porokeratosis, and porokeratosis palmaris et plantaris disseminata. In 1995, Lucker et al. (1) described a subtype of porokeratosis: porokeratosis ptychotropica (PP). Clinically, this lesser-known entity most commonly affects body folds, especially the gluteal cleft, with or without involvement of the extremities. Male genitalia involvement is rare (2). We report here a special case of PP presenting with pruritic brownish verrucous plaques on the penis and scrotum.

Porokeratosis Ptychotropica on the Penis and Scrotum: A Case Report

Epidermal growth factor receptors (EGFRs) regulate the growth and repair process of epithelia, as well as carcinogenesis. Activation of TRPV3 by carvacrol stimulates skin inflammation and epidermal hyperplasia; the latter can be suppressed by EGFR inhibition. However, whether EGFR signalling is responsible for skin inflammation remains elusive. The current study investigated the effect of erlotinib, an EGFR inhibitor, on skin inflammation in a carvacrol-induced atopic dermatitis mouse model. It was observed that erlotinib significantly attenuated carvacrol-induced overexpression of proinflammatory cytokines and suppressed peripheral blood mononuclear cell recruitment in HaCaT keratinocytes. In addition, it was demonstrated that erlotinib suppressed carvacrol-induced Akt and NF-κB signalling pathways. Furthermore, inhibition of Akt and NF-κB signalling pathways also attenuated the carvacrol-induced keratinocyte proinflammatory response. Finally, it was demonstrated that erlotinib treatment alleviated carvacrol-induced dermatitis. These data demonstrate that erlotinib ameliorates skin inflammation by regulating Akt and NF-κB-mediated keratinocyte proinflammation, suggesting the therapeutic potential of erlotinib, a clinically used EGFR inhibitor, in skin inflammatory diseases.

A Case of Pemphigus Herpetiformis Strongly Associated with Gastric Cancer

Pyoderma gangrenosum is a rare, autoinflammatory disorder characterized by rapidly progressive painful ulcers that are challenging to diagnose and treat. This qualitative study aimed to explore the experiences of patients living with pyoderma gangrenosum. Using an inductive qualitative approach, semi-structured interviews were completed with a purposive sample of 21 patients with pyoderma gangrenosum recruited from a public dermatology outpatient clinic in Melbourne, Australia. A reflexive thematic analysis was performed, yielding 5 themes: pain, physical challenges, social functioning and relationships, mental health, and treatment. The impact of pyoderma gangrenosum on quality of life was multifaceted and varied throughout disease progression, remission, and recurrence. Experiences of delayed diagnosis and misdiagnosis were common, causing distress and resulting in unnecessary treatments including surgery. Severe pain disrupted sleep and limited daily activities, eroding patients’ sense of self-control and perpetuating depressed mood and anxiety. Management should include early specialist referral, providing information sheets for managing pain and wound care, and communicating disease expectations. In conclusion, this study has deepened understanding and given personal perspectives on what it is like to live with a condition poorly understood by many health professionals. Increased efforts should be made to increase clinician awareness regarding pyoderma gangrenosum to facilitate early diagnosis.
