Intra‐articular Injections of Osmic Acid in Painful Joint Affections

A 58-year-old patient with neutropenia due to SLE developed spondylitis of the lumbar region caused by Candida albicans. The spondylitis was probably superinfected with Staphylococcus aureus. The initial one month's intravenous combination therapy with amphotericin B and flucytosine was discontinued because of fever reactions to amphotericin B, suspected myelosuppressive effect of flucytosine and insufficient clinical response. This therapy was followed by four months of oral ketoconazole and clindamycin with good results and without any side-effects.

Abstract. Insulin‐induced antilipolysis was investigated in fat cells obtained after an overnight fast and 60 min after glucose ingestion in seven non‐obese patients with non‐insulin‐dependent diabetes mellitus (NIDDM). The study was performed before and after long‐term therapy with diet and glibenclamide. After treatment, the antilipolytic potency of insulin in fat cells was threefold enhanced ( p &lt;0.05) in the fasting state and remained unaltered after glucose ingestion. In untreated NIDDM oral glucose induced a significant ( p &lt;0.01) increase in insulin sensitivity. In consequence, in the glucose‐fed state insulin sensitivity was similar before and after therapy. Adipocyte insulin receptor binding was comparable before and after therapy, both in the fasting state and following glucose intake. In untreated NIDDM, despite relative hypoinsulinemia, plasma glycerol was markedly reduced after oral glucose. After therapy, plasma glycerol was significantly reduced both in the fasting state and following glucose ingestion. At the same time, fasting and glucose‐stimulated circulating insulin were significantly ( p &lt;0.01) increased. It is concluded that conventional antidia‐betes therapy in NIDDM mediates a suppression of adipose tissue lipolysis. This seems to be due to an improvement in insulin secretion in combination with a potentiation of the antilipolytic effectiveness of insulin in fat cells in the fasting state, the latter being secondary to post‐binding alterations in insulin action.

ABSTRACT A high fibre and a low fibre breakfast meal were given to eight non insulin dependent diabetics (NIDD), and eight controls. Blood glucose response was monitored continuously for three hours and characterized using a straight line model. After the high fibre meal the rates of increase and decrease in blood glucose concentration were slower both in diabetics and controls than after the low fibre meal. The delay time, however, i.e. the time from meal intake to the start of glucose increase, hypothetically corresponding to gastric emptying time, was the same after both test meals. The postprandial glucose increment calculated as the area under the 0–120 min curve was lower after the high fibre meal in the NIDD, but not in the controls. The two-hour C-peptide and gastric inhibitory polypeptide values were lower for the diabetics after the high fibre breakfast. The results indicate a prolonged carbohydrate digestion and/or absorption after high fibre breakfast.

Effect of Dietary Fibre on Blood Glucose, Plasma Immunoreactive Insulin, C-peptide and GIP Responses in non Insulin Dependent (Type 2) Diabetics and Controls

ABSTRACT Sixty-four patients over the age of 40 years, undergoing elective surgery of at least one hour's duration, were randomized to treatment with either a thromboembolic deterrent (TED) stocking (Kendall Co.) or subcutaneous low-dose heparin 5000 IU every 12 hours. Serum levels of alanine aminotransferase (S-ALAT), aspartate aminotransferase (SASAT), γ-glutamyl transpeptidase (S-γ-GT) and alkaline phosphatase (S-ALP) were measured. S-ALAT increased significantly on the 5th and 10th postoperative day, from 27±2 (x̄±SE) to 40±4 (p<0.01) and 55±7 U/1 (p<0.001), respectively, in the heparin group and was significantly higher in the heparin than in the TED group both on the 5th (p<0.01) and 10th (p<0.05) postoperative day. S-ASAT and S-γ-GT increased significantly during heparin treatment, but did not differ significantly from the values of the TED group. No change in S-ALP was registered in either group. It is concluded that prophylactic treatment with low-dose heparin induces a significant increase in S-aminotransferase levels, especially in S-ALAT. The phenomenon has profound differential diagnostic implications in conditions such as pulmonary embolism and acute myocardial infarction.

Heparin-Induced Increase in Serum Levels of Aminotransferases

ABSTRACT Eleven patients with recurrent venous thrombosis or thrombophlebitis and a decreased fibrinolytic activity after venous occlusion were given a phenformin-like substance, moroxydine chloride, for 6 months. Strokes occurring in persons 15-65 years of age (population 300 000) have been registered since 1970 in Göteborg, Sweden. From 1971 to 1974, 640 patients were registered and followed for at least one year. Coexisting cardio-cerebrovascular diseases were more frequent in the stroke patients than in random samples of the population of Göteborg of similar ages for each sex. At three weeks the degree of neurological deficit and type of stroke were decisive for prognosis, while the presence of cardio-cerebrovascular diseases influenced fatality rates negatively at one year.
