Retrospective study to evaluate the efficacy and safety of endoscopic stapled diverticulotomy of patients with Zenker's diverticulum.Twenty-two patients with Zenker's diverticulum were evaluated. All patients had a preoperative barium swallow, which confirmed the Zenker diverticulum.Patients underwent an attempted endoscopic resection of the Zenker diverticulum using the endoscopic stapling technique. If unsuccessful, an open approach was then taken. All patients were seen in follow-up within one and then at six months after surgery and had a barium swallow during the first month.The patient's mean age was 74 years. The operation lasted a mean of 20minutes. Conversion to open surgery was required in five patients, which lasted a mean 20minutes. No postoperative morbidity or mortality was recorded. In two patients with a small diverticulum (2cm), persistent discomfort with no dysphagia or regurgitation was noted. The barium swallow demonstrated a persistent diverticulum without a neck.Endoscopic staple diverticulotomy is an excellent first-intention method to surgically correct Zenker's diverticulum in many patients. It is a technique with a significantly shorter operative time, hospital stay, time to resumption of oral feeding and lower mortality and fewer morbidity complications.

Traitement endoscopique des diverticules de Zenker à la pince autosuturante

Décrire le cas d’un carcinome mucoépidermoïde de la parotide occasionnant une obstruction d’un canal excréteur salivaire et se manifestant par un kyste parotidien de type rétentionnel. Une patiente âgée de 21 ans consulte pour une tuméfaction de la loge parotidienne gauche, d’allure kystique, de 5 cm de diamètre. L’échographie et la tomodensitométrie objectivent une formation kystique parotidienne. Un abord de parotidectomie est réalisé avec exérèse du kyste et de tissu parotidien autour de la lésion. Les suites opératoires sont simples. L’examen anatomopathologique de la pièce opératoire objective un kyste parotidien de type rétentionnel et un carcinome mucoépidermoïde de 5 mm de diamètre. Parmi les kystes acquis de la parotide, les kystes de type rétentionnel sont les plus fréquents. Ils peuvent être la manifestation d’un microcarcinome de la parotide. Dans ce cas clinique, le carcinome mucoépidermoïde en obstruant un canal excréteur, a été responsable de la formation d’un kyste rétentionnel. Une exérèse au large des kystes de type rétentionnel est nécessaire, ainsi qu’un examen anatomopathologique minutieux à la recherche d’une lésion maligne. To describe a case of mucoepidermoid carcinoma of the parotid gland that caused obstruction of a salivary gland duct and manifested as a parotid retention cyst. A 21-year-old woman presented with a 5-cm cystic swelling in her left parotid region. Ultrasound examination and a computerized tomography scan showed a clearly outlined parotid cyst. Through a conventional parotidectomy incision, the cyst was excised with parotid gland tissue around the lesion. The patient had an uncomplicated recovery. Histopathological examination of the resected parotidectomy material showed a parotid retention cyst and a mucoepidermoid carcinoma measuring 5 mm in diameter. Among acquired cysts of the parotid gland, retention cysts are the most frequent. They may be the presenting symptom in patients with occult cancer of the parotid. In this case report, a mucoepidermoid carcinoma played a significant role in the obstruction of a salivary gland duct and caused a parotid retention cyst. Thus, a wide excision of the parotid retention cyst is necessary, as is a careful review of the histology searching for malignant lesion.

Kyste parotidien rétentionnel révélateur d’un carcinome mucoépidermoïde

Le but de notre étude rétrospective préliminaire est d’évaluer la valeur pronostique de l’infiltration tumorale dans les carcinomes épidermoïdes du plancher buccal. Cette étude préliminaire inclut 61 patients atteints d’un carcinome épidermoïde du plancher buccal classé T1N0, T2N0 ou T3N0 traités en première intention à but curative entre 1995 et 2005. L’infiltration tumorale a été définie histologiquement. Les probabilités de survie à deux et cinq ans ont été étudiées en fonction de l’infiltration tumorale. La population est constituée de 52 hommes et de neuf femmes. L’âge moyen était de 58,1 ans. L’infiltration tumorale moyenne et médiane était, respectivement, de 7,2 et 6 mm. Le temps de survie moyen était de 56 mois. Les probabilités de survie globale à deux et cinq ans étaient de 79,5 et 36,7 %, respectivement. Les probabilités de survie globale et de survie sans récidive à deux et cinq ans ne variaient pas de façon significative en fonction du degré de l’infiltration tumorale. Cette étude préliminaire ne nous a pas permis de démontrer la valeur pronostique de l’infiltration tumorale au cours des carcinomes épidermoïdes du plancher buccal. Dans l’attente des résultats d’une étude incluant un plus grand nombre de patients, il nous semble cependant nécessaire de prendre en compte la valeur de l’infiltration tumorale au cours de la stratégie thérapeutique des carcinomes épidermoïdes de la cavité buccale. The aim of this preliminary retrospective study was to evaluate the prognostic value of tumor thickness in squamous cell carcinoma of the floor of the mouth. A total of 61 patients with previously untreated T1N0, T2N0 or T3N0 squamous cell carcinoma of the floor of the mouth were included in this preliminary study. An optical micrometer was used to determine the maximum of tumor thickness. We studied the relation between the overall survival at two and five years and tumor thickness. The cohort comprised 52 male and nine female patients. The mean and the median thickness were 7.2 and 6 mm, respectively. Overall survival was 79.5 and 36.7% at two and five years, respectively. There was no significant association between thickness and overall survival (p = 0.71) and between thickness and disease-free survival (p = 0.63). The prognostic value of tumor thickness was not demonstrated in this preliminary study. We are currently conducting a study involving a larger patient population in our unit and we believe that tumor thickness should be considered in the management plan for patients with oral carcinoma.

Valeur pronostique de l’infiltration des carcinomes épidermoïdes du plancher buccal

La localisation maxillofaciale du kyste hydatique est rare et représente 2 % des cas. L’objectif de ce travail est de rappeler les signes cliniques et radiologiques en insistant sur les difficultés diagnostiques. Nous présentons deux observations de kyste hydatique à localisation maxillofaciale. Les deux cas ont été explorés par un panoramique dentaire, une échographie et une tomodensitométrie. L’imagerie par résonance magnétique a été pratiquée dans un cas. Dans la première observation, il s’agit d’une jeune fille de 23 ans qui a consulté pour une tuméfaction jugale à extension pharyngienne évoluant par poussée. La deuxième observation est celle d’une jeune fille de 16 ans qui a consulté pour une tuméfaction génienne et massétérine avec un trismus serré. Dans les deux cas, l’imagerie a suspecté le diagnostic du kyste hydatique et l’examen anatomopathologique l’a confirmé. Le kyste hydatique est une pathologie parasitaire endémique qui touche surtout le poumon et le foie. Sa localisation au niveau de la sphère maxillofaciale est exceptionnelle. Devant une masse kystique dans une zone endémique, le diagnostic du kyste hydatique doit être envisagé. La TDM et l’IRM permettent un bilan lésionnel précis. Le traitement est chirurgical. A maxillofacial location of a hydatid cyst is rare, accounting 2% of cases. The purpose of this study was to review clinical and radiological features by emphasizing diagnostic difficulties. Two observations of hydatid cyst with a maxillofacial location are presented. Two cases were investigated by panoramic dental radiography, an ultrasound and CT scan. MRI was done in one case. The first observation reports a 23-year-old woman who consulted for jugal tumefaction with pharyngeal extension evolving in spurts. The second observation was a 16-year-old girl, which consulted for a genian and masseter tumefaction with a tight trismus. In both cases, the imaging studies suggested the diagnosis of the hydatid cyst confirmed by the anatomopathologic exam. The hydatid cyst is an endemic parasitic pathology involving most often the lung and the liver. The maxillofacial location is exceptional. In cases of a cyst mass in an endemic zone, the diagnosis of the hydatid cyst must be entertained. CT scan and MRI provided a complete lesion workup. Treatment is surgical.

Rapporter un cas de tuberculose laryngée et rappeler la prise en charge sanitaire de la pathologie tuberculeuse en pratique ORL. Un patient, avec un antécédent de tabagisme, suivi pour une sarcoïdose pulmonaire, a été adressé pour dysphonie et dysphagie. L'examen ORL a révélé une ulcération isolée de la face laryngée de l'épiglotte. Cette ulcération faisait évoquer une sarcoïdose laryngée, un carcinome ou une tuberculose épilaryngée. Une pan-endoscopie des voies aérodigestives supérieures a permis de diagnostiquer une granulomatose gigantocellulaire avec nécrose caséeuse. La culture des biopsies a identifié un Mycobacterium tuberculosis. Le traitement antituberculeux institué a permis la disparition des symptômes ORL. L'ulcération épiglottique a disparu à trois mois de traitement. La régression des images pulmonaires faisait suspecter une tuberculose pulmonaire ancienne associée. La découverte de ce cas de tuberculose laryngée a permis de mettre en place une surveillance sanitaire intra- et extrahospitalière. La tuberculose épilaryngée doit être évoquée en cas de lésion atypique. La déclaration obligatoire permet de mettre en place les enquêtes sanitaires. To report a case of laryngeal tuberculosis and to consider tuberculosis management in ENT practice. A 44-year-old man, a smoker with pulmonary sarcoidosis experienced dysphonia with dysphagia. Laryngeal fibroscopy revealed an ulcerated epiglottic lesion. Direct laryngoscopy was performed to detect carcinoma, laryngeal sarcoidosis or tuberculosis. The histologic study revealed granulomatosis with giant cells and caseous necrosis. Tissue culture identified Mycobacterium tuberculosis. Antituberculosis therapy decreased dysphonia and dysphagia. Isolated ulceration disappeared at three months. Pulmonary infiltration decreased in radiography. The discovery of this case of laryngeal tuberculosis instigated hospital and community tuberculosis surveillance. The ENT specialist should be aware of laryngeal tuberculosis in suspicious lesions. Mandatory declaration of such cases can motivate setting up tuberculosis surveillance.

Évaluer l’apport d’un logiciel original de reconstruction 3D basé sur le principe de la « croissance de région » à partir d’images tomodensitométriques, au diagnostic des malformations des osselets de l’oreille moyenne. Étude prospective sur une population de dix patients présentant une surdité de transmission uni- ou bilatérale. Vingt oreilles ont été étudiées par scanner multicoupes. Toutes les données ont été traitées par un logiciel original de segmentation d’images basée sur le principe de la « croissance de région ». Les images 3D ont été comparées avec les coupes originales de scanner afin d’en juger l’apport potentiel à la prise en charge de ces patients. Toutes les images obtenues ont été jugées de qualité diagnostique suffisante. Les deux oreilles normales sur les coupes scanner l’étaient également sur les vues 3D. Des images scanner typiques d’otospongiose retrouvées chez deux patients n’avaient aucune traduction sur les images 3D. Les 18 malformations ossiculaires ont bénéficié d’une bonne analyse sur les images obtenues ; ces dernières permettant en outre une meilleure visualisation des anomalies et des lésions associées (en particulier les anomalies de position du nerf facial) permettant ainsi un planning opératoire jugé de bonne qualité par le chirurgien. La méthode de reconstruction d’images étudiée, par « croissance de région », nous semble, sur cet échantillon limité, être un complément utile aux images tomodensitométriques classiques, dans l’étude des anomalies constitutionnelles de l’oreille moyenne. Evaluate the contribution of original 3D reconstruction software based on the principle of region growing segmentation of CT images to diagnosis of ossicular malformations of the middle ear. Prospective study on a population of ten patients presenting uni- or bilateral transmission deafness. Twenty ears were studied using multislice CT. All the data were treated using original software segmentation based on the principle of region growing segmentation. The 3D images were compared with the original CT slices to judge the software's potential contribution to the management of these patients. All the images obtained were deemed of sufficient diagnostic quality. The two normal ears on the CT slices were also found to be normal on the 3D images. Typical CT images of otospongiosis found in two patients were not visible on the 3D images. The 18 ossicular malformations were well analyzed on the images obtained, which also provided better visualization of the abnormalities and associated lesions (particularly the anomalies in facial nerve position), thus providing quality surgical planning as judged by the surgeon. The image reconstruction method studied, using region growing segmentation seems to be, in this limited sample, a useful complement to classical CT images in the study of constitutional abnormalities of the middle ear.

L’objectif de cet article est de rapporter les principales études et les solutions actuelles pour remplacer, grâce à des techniques de reconstruction, la trachée. Cette revue de la littérature a été conduite en sélectionnant les articles et les références des articles revus en utilisant parmi les moteurs de recherche les mots clés reconstruction de trachée, greffes et ingénierie tissulaire mais également d’après l’expérience des auteurs dans ce domaine. Les reconstructions de trachée sans utilisation de matériaux étrangers font principalement appel aux greffes tissulaires (auto- ou allogreffe) et à l’ingénierie tissulaire. Parmi de nombreux procédés déjà décrits, trois nouvelles techniques ont émergé ces dernières années utilisant des cellules souches mésenchymateuses autologues, des cultures de cellules épithéliales et une matrice dérivée d’un greffon trachéal ; des prélèvements de cartilage et de muqueuse associés à des lambeaux locaux ou à distance ; des greffons d’aorte. Ces reconstructions ont été réalisées chez l’homme avec des résultats apparemment satisfaisants mais avec un recul encore limité. Les techniques de reconstructions tissulaires ont largement progressé permettant d’envisager dans l’avenir le remplacement de longs segments de trachée. Ces reconstructions associées au développement des biomatériaux pourraient, par ailleurs, faciliter la conception et l’insertion d’une prothèse laryngée. To review the main studies and the recent surgical procedures in tracheal reconstruction. The literature search was conducted using the key words “tracheal reconstruction”, “grafts”, and “tissue engineering” and by selecting references from the articles reviewed as well as the experience of the authors in this field. Surgical reconstruction for tracheal replacement without using biomaterials involves tissue grafts (auto- or allografts) and tissue engineering. Among the many procedures already described, three new techniques have emerged these past few years employing autologous mesenchymal stem-cell-derived chondrocytes, autologous cultured epithelial cells, and a matrix derived from tracheal graft; costal cartilage, recipient mucosa, and local or free flaps, and an aortic graft. These procedures have been proposed in humans with apparently good results but with a still limited follow-up. Tracheal reconstruction techniques have recently progressed and replacing a long segment of trachea can be envisaged for the future. Moreover, these reconstructions, in conjunction with biomaterial development, would facilitate the design and the implantation of a laryngeal prosthesis.

Illustrer à travers ce cas clinique, la rareté et la difficulté du diagnostic du neurofibrome du sinus maxillaire. Une patiente de 35 ans consultait pour une tuméfaction de la joue gauche évoluant depuis six mois, tuméfaction de la gencive supérieure gauche et un trouble de l’articulé dentaire. À l’examen tomodensitométrique du massif facial, on notait un processus tumoral du sinus maxillaire gauche détruisant les parois. L’examen histologique d’un fragment de biopsie évoquait un carcinome in situ. La patiente a bénéficié d’une maxillectomie. L’examen anatomopathologique de la pièce opératoire mettait en évidence un neurofibrome. Elle a été suivie pendant huit mois sans récidive. La difficulté du diagnostic du neurofibrome du sinus maxillaire est liée à l’absence de signes cliniques et radiologiques spécifiques. Donc les oto-rhino-laryngologistes doivent avoir en arrière pensée cette variété histologique rare dans la panoplie des tumeurs des sinus paranasaux. To illustrate the rarity and difficulty diagnosing maxillary sinus neurofibroma through a case report. A 35-year-old female consulted our department for left cheek swelling evolving over 6 months, upper gum swelling, and a dental occlusion disorder. A computed tomography scan showed a tumor of the left maxillary sinus with bone destruction. Histological examination of a biopsy fragment found an in situ carcinoma. A maxillary resection was performed to excise the tumor. Histological examination of the specimen showed a neurofibroma. No sign of recurrence was noted after 8 months of follow-up. The difficulty diagnosing maxillary sinus neurofibroma is related to its nonspecific clinical and radiological signs. Consequently, the otorhinolaryngologist must keep this rare histological variety in mind within the range of tumors of the paranasal sinuses.
