Alzheimer’s disease (AD) is a progressive neurological disease. Gut microbial dysbiosis is associated with AD. This study involves the comparative assessment of spatial learning, β-amyloid peptide accumulation, and fecal microbiota alterations in 3×Tg-AD mice from 3 age groups: AD asymptomatic stage (3 m), presymptomatic stage (6 m), and the symptomatic stage of AD (9 m). We demonstrate that spatial memory deficits, brain Aβ accumulation, and weight gain in 3×Tg-AD mice gradually appear after 6 months of age. However, the total gut bacterial counts underwent changes from 3 to 6 months of age and were further altered at 9 months of age. Importantly, changes in gut bacteria abundance of Desulfobacterota and Actinobacteriota phyla in 6-month-old mice preceded apparent spatial memory deficits. In summary, Changes in the gut microbial community are one of the mechanisms of early AD pathology.

Alterations of Spatial Memory and Gut Microbiota Composition in Alzheimer’s Disease Triple-Transgenic Mice at 3, 6, and 9 Months of Age

The spectrum of cognitive decline in the elderly ranges from what can be classified as normal cognitive decline with aging to subjective cognitive impairment to mild cognitive impairment (MCI) to dementia. This article reviewed the up-to-date evidence of MCI including the diagnostic criteria of MCI due to Alzheimer’s disease, vascular cognitive impairment and MCI due to Parkinson disease, management and preventive intervention of MCI. There are various etiologies of MCI, and a large number of studies have been conducted to ascertain the practical modalities of preserving cognition in predementia stages. Lifestyle modification, such as aerobic exercise, is an approved modality to preserve cognitive ability and decrease the rate of progression to dementia, as well as being recommended for frailty prevention.

Mild Cognitive Impairment in Clinical Practice: A Review Article

Amnestic mild cognitive impairment (aMCI) and vascular cognitive impairment with no dementia (VCIND) are highly predictive of Alzheimer’s disease and vascular dementia. In this study, a 2-dimensional magnetic resonance spectroscopy was performed in 25 patients with aMCI, 28 patients with VCIND, and 32 normal controls (NCs). The concentrations of N-acetyl aspartate (NAA), choline (Cho), myoinositol (MI), and creatine (Cr) were measured, and their ratios were calculated. The patients with aMCI displayed significantly lower NAA/MI bilaterally in the posterior cingulate gyrus (PCG) and white matter of occipital lobe (OLWM) than NC participants or patients with VCIND , whereas patients with VCIND displayed markedly lower NAA/Cho bilaterally in the white matter of frontal lobe (FLWM) and left OLWM, and right dorsal thalamus (DT) than patients with NC or aMCI. Compared with the controls, patients with aMCI displayed lower NAA and NAA/Cr in bilateral PCG, left precuneus, and DT, whereas patients with VCIND displayed lower NAA/Cr in bilateral DT and FLWM. In addition, increased MI in right PCG of patients with aMCI and increased Cho in left FLWM of patients with VCIND were also observed. The results might help guide a clinical differentiation between the 2 disorders.

Magnetic Resonance Spectroscopy Study of Amnestic Mild Cognitive Impairment and Vascular Cognitive Impairment With No Dementia

Objective To evaluate the effects of light therapy on the alleviation of sleep disturbances, agitation and depression in people with dementia. Methods A search was performed in PubMed, Medline, SCOPUS, Web of Science, EMBASE, CINAHL, Cochrane Library, for studies published between 2000 and 2021. Results A total of 4315 articles were screened. Sixteen articles were eligible for this review and 11 randomized controlled studies were included in the meta-analysis. Light therapy had a significant effect on reducing the number of awakenings in sleep ( n = 4; 95% CI = −.56, −.05; I 2 = 0%; SMD = −.31) but was not significant in reducing the wake after sleep onset ( n = 3; 95% CI = −.14, .59; I 2 = 0%; SMD = .23), agitation ( n = 4; 95% CI = −1.02, .45; I 2 = 87%; SMD = −.28) and depression ( n = 6; 95% CI = −.80, .40, I 2 = 85%; SMD = −.20). Conclusion Light therapy appeared to be more effective in terms of alleviating sleep disturbances, rather than reducing agitation and depression, but its long-term effects remain unclear.

BESI: Behavioral and Environmental Sensing and Intervention for Dementia Caregiver Empowerment—Phases 1 and 2

Background: Alterations in Heart Rate (HR) and Heart Rate Variability (HRV) reflect autonomic dysfunction associated with neurodegeneration making them biomarkers suitable for detecting Mild Cognitive Impairment (MCI). Methods: The study involves 297 urban Indian participants [48.48% (144) were male and 51.51% (153) were female]. MCI was detected in 19.19% (57) of participants and the rest, 80.8% (240) of them were healthy. ECG recordings spanning 10 s were collected and R-peaks were detected. Machine learning algorithms like were employed to further validate the features. Results: The mean of R-to-R (NN) intervals ( P = .0021), the RMS of NN intervals ( P = .0014), the SDNN ( P = .0192) and the RMSSD ( P = .0206) values differ significantly between MCI and non-MCI. Machine learning classifiers, SVM, DA, and NB show a high accuracy of 80.801% on RMS feature input. Conclusion: HR and its variability can be considered potential biomarkers for detecting MCI.

Heart Rate and its Variability From Short-Term ECG Recordings as Potential Biomarkers for Detecting Mild Cognitive Impairment

Timely diagnosis of young-onset dementia (YOD) is critical. This study aimed to identify factors that increased time to diagnosis at each stage of the diagnostic pathway. Participants were patients diagnosed with YOD (n = 40) and their care partners (n = 39). Information was obtained from questionnaires, and review of medical records. Mean time from symptom onset to YOD diagnosis was 3.6 ± 2 years. Suspicion of depression/anxiety at presentation was associated with significantly increased time from presentation to specialist referral. Neurologist-diagnosed YOD was the fastest route to a diagnosis, whereas diagnoses made by other specialists significantly increased the time from first specialist visit to diagnosis. By investigating multiple stages of the diagnostic pathway, we identified two factors that increased time to diagnosis: suspicion of depression/anxiety at presentation delayed specialist referral from primary care, and diagnosis by a specialist other than a neurologist delayed diagnosis of YOD.

Determinants of Time to Diagnosis in Young-Onset Dementia

Several research studies have demonstrated the potential use of cerebrospinal fluid biomarkers such as amyloid beta 1-42, T-tau, and P-tau, in early diagnosis of Alzheimer's disease stages. The levels of these biomarkers in conjunction with the dementia rating scores are used to empirically differentiate the dementia patients from normal controls. In this work, we evaluated the performance of standard machine learning classifiers using cerebrospinal fluid biomarker levels as the features to differentiate dementia patients from normal controls. We employed various types of machine learning models, that includes Discriminant, Logistic Regression, Tree, K-Nearest Neighbor, Support Vector Machine, and Naïve Bayes classifiers. The results demonstrate that these models can distinguish cognitively impaired subjects from normal controls with an accuracy ranging from 64% to 69% and an area under the curve of the receiver operating characteristics between 0.64 and 0.73. In addition, we found that the levels of 2 biomarkers, amyloid beta 1-42 and T-tau, provide a modest improvement in accuracy when distinguishing dementia patients from healthy controls.

A Study on Machine Learning Models in Detecting Cognitive Impairments in Alzheimer’s Patients Using Cerebrospinal Fluid Biomarkers

Alzheimer’s disease (AD) is the most common cause of dementia. Epileptic seizures or epileptic activity have been detected in AD, and people with epilepsy have a higher risk of dementia compared to the general population. This suggests that seizures or epileptic activity might often coexist with AD. It is increasingly evident that epileptic activity or seizures are common but often overlooked comorbidities of AD. However, the causal relationship between seizures, epileptic activity and cognitive decline remains uncertain. Experimental data show that amyloid-β (Aβ) and Tau protein can cause neuronal hyperexcitability and has epileptogenic effects. Neural network hyperexcitation regulates the ratio of Aβ isoforms and is linked to the initiation of AD, indicating a shared mechanism. Clinical studies suggest that cognitive impairment accelerates in AD patients with seizures or epileptic activity. This review discusses the relationship between epileptic seizures and AD, the impact of epileptic activity in AD, and potential treatments.

The Role of Epileptic Activity in Alzheimer’s Disease

The study aims to identify psychological symptoms (depression and anxiety) and their relationship to the quality of life among dementia patients’ caregivers, and whether there are differences in the level of each of them due to the gender variable. The study follows the correlational approach, with a sample of 174 dementia patients’ caregivers. To pursue the analysis, the study uses 3 measurement tools: anxiety, depression, and quality of life. The results show that the level of depression, anxiety, and quality of life among dementia patients’ caregivers is moderate. It also finds that there is a positive relationship between anxiety and depression, and there is a negative relationship between quality of life and anxiety and depression. There are no differences in the level of depression and anxiety due to gender, as the study finds female caregivers to have a higher level of quality of life.
