To propose a common nomenclature to refer to individuals who fulfill the American College of Rheumatology classification criteria for systemic lupus erythematosus (SLE) during childhood or adolescence.The medical literature was reviewed for studies conducted in the target population between 1960 and December 2011 to obtain information about the terms used to refer to such children and adolescents. We reviewed the threshold ages used and disease features considered to discriminate these individuals from patients with onset of SLE during adulthood. Furthermore, the nomenclature used in other chronic diseases with onset during both childhood and adulthood was assessed.There was an astonishing variability in the age cutoffs used to define SLE onset prior to adulthood, ranging from 14-21 years, but most studies used age 18 years. The principal synonyms in the medical literature were SLE without reference to the age at onset of disease, childhood-onset SLE, juvenile SLE, and pediatric (or paediatric) SLE.Based on the definition of childhood, in analogy with other complex chronic diseases commencing prior to adulthood, and given the current absence of definite genetic variations that discriminate adults from children, the term childhood-onset SLE is proposed when referring to individuals with onset of SLE prior to age 18 years.

Objective Little is known of the clinical features and comorbidity profile of patients presenting with early‐onset gout (EOG), although international guidelines recommend rapid treatment after diagnosis. The objective of this study was to assess specific characteristics and comorbidities of patients with gout who had an early onset. Methods Patients from a cross‐sectional French national cohort who experienced their first gout flare before age 40 years were included in the EOG group and compared to patients with an onset after age 40 years, the common gout group. Results A total of 120 patients were included in the EOG group (mean ± SD age 49.5 ± 11.9 years) and 865 patients in the common gout group (mean ± SD age 64.4 ± 10.1 years). Patients with EOG more often presented with a history of polyarticular flares ( P &lt; 0.01), but had similar frequency of flares ( P = 0.16), gout arthropathy ( P = 0.79), and tophi ( P = 0.53). Prevalence of each item comprising metabolic syndrome did not differ between groups. In patients with EOG, all cardiovascular comorbidities were diagnosed after gout onset. Greater age, low high‐density lipoprotein, and excessive alcohol intake were associated in multivariate analysis with the common gout group, while a familial history of gout, longer duration of urate‐lowering treatment, higher serum uric acid levels, and metabolic syndrome were associated with the EOG group. Conclusion Patients with EOG developed slightly more severe joint involvement and earlier metabolic disorders than patients with common gout.

Patients With Early‐Onset Gout and Development of Earlier Severe Joint Involvement and Metabolic Comorbid Conditions: Results From a Cross‐Sectional Epidemiologic Survey

Abstract Objective To determine the prevalence, interrelation, and impact of musculoskeletal disorders of the upper limb in the general population. Methods A total of 9,696 randomly selected adults of working age were surveyed in a 2‐stage cross‐sectional study involving a screening questionnaire and a standardized physical examination in symptomatic subjects. Age‐ and sex‐specific prevalence rates were estimated for several musculoskeletal disorders and for nonspecific pain in the upper limbs. The overlap and impact on daily activities and healthcare utilization were explored. Results Among 6,038 first‐stage responders, 3,152 reported upper limb symptoms and 1,960 were subsequently examined. Of subjects with pain, 44.8% had 1 or more specific soft‐tissue disorders. Site‐specific prevalence rates were as follows: shoulder tendinitis 4.5% among men and 6.1% among women; adhesive capsulitis 8.2% among men and 10.1% among women; lateral epicondylitis 1.3% among men and 1.1% among women; de Quervain's disease 0.5% among men and 1.3% among women; other tenosynovitis of the hand or wrist, 1.1% among men and 2.2% among women. Specific disorders tended to cluster ( P &lt; 0.001) in individuals, with particular overlap at the shoulder. Compared with subjects with nonspecific pain, those with specific disorders more often reported inability to perform everyday tasks ( P &lt; 0.05), consultation with a doctor ( P &lt; 0.05), and use of prescribed medication ( P &lt; 0.05). Conclusion Upper limb pain is common in the general population and is often associated with physical signs suggestive of specific upper‐limb disorders. These disorders have a substantial impact on physical function and use of health care.

Prevalence and impact of musculoskeletal disorders of the upper limb in the general population

Objective To investigate the efficacy and safety of creatine supplementation in fibromyalgia patients. Methods A 16‐week, randomized, double‐blind, placebo‐controlled, parallel‐group trial was conducted. Fibromyalgia patients were randomly assigned to receive either creatine monohydrate or placebo in a double‐blind manner. The patients were evaluated at baseline and after 16 weeks. Muscle function, aerobic conditioning, cognitive function, quality of sleep, quality of life, kidney function, and adverse events were assessed. Muscle phosphorylcreatine content was measured through 31 P magnetic resonance spectroscopy. Results After the intervention, the creatine group presented higher muscle phosphorylcreatine content when compared with the placebo group (+80.3% versus −2.7%; P = 0.04). Furthermore, the creatine group presented greater muscle strength than the placebo group in the leg press and chest press exercises (+9.8% and +1.2% for creatine versus −0.5% and −7.2% for placebo, respectively; P = 0.02 and P = 0.002, respectively). Isometric strength was greater in the creatine group than in the placebo group (+6.4% versus −3.2%; P = 0.007). However, no general changes were observed in aerobic conditioning, pain, cognitive function, quality of sleep, and quality of life. Food intake remained unaltered and no side effects were reported. Conclusion Creatine supplementation increased intramuscular phosphorylcreatine content and improved lower‐ and upper‐body muscle function, with minor changes in other fibromyalgia features. These findings introduce creatine supplementation as a useful dietary intervention to improve muscle function in fibromyalgia patients.

Advancing Rheumatologic Care in Older Adults: Highlights from the 2024 American Geriatrics Society Annual Scientific Meeting

There are over 100 rheumatic diseases and approximately 300,000 children with a pediatric rheumatic disease (PRD) in the United States (U.S.). The most common PRDs are juvenile idiopathic arthritis (JIA), childhood‐onset systemic lupus erythematosus (cSLE), and juvenile dermatomyositis (JDM). Effective and safe medications are essential because there are generally no cures for these conditions. Etanercept was the first biologic therapy for the treatment of JIA, approved in 1999. Since then, other biologic disease modifying antirheumatic drugs (bDMARDs) and targeted synthetic DMARD (tsDMARDs) blocking relevant immunological pathways have been approved for the treatment of JIA, resulting in a marked improvement of disease prognosis. Conversely, there is only one bDMARD that has been approved for cSLE, but none are approved for the treatment of JDM. Lack of approved therapeutic options, with established dosing regimens and known efficacy and safety, remains a central challenge in the treatment of all PRDs, including autoinflammatory diseases, and for complications of PRDs. This review provides an overview of bDMARD and tsDMARD treatments studied for the treatment of various subtypes of JIA, summarizes information from bDMARD studies in other pediatric rheumatic diseases, with a focus on pivotal trials that led to regulatory approvals and highlights improved outcomes in JIA with the use of these newer medications. Further, we outline barriers and challenges in the treatment of other PRDs. Lastly, we summarize the current regulatory landscape for bDMARD studies and medication approvals in PRDs.

25 Years of Biologics For The Treatment of Pediatric Rheumatic Diseases – Advances in Prognosis and Ongoing Challenges

Objective Patients with rheumatoid arthritis (RA) are at increased risk of cardiovascular disease (CVD) including heart failure (HF). However, little is known regarding the relative risks of heart failure subtypes such as HF with preserved (HFpEF) or reduced ejection fraction (HFrEF) in RA compared to non‐RA. Methods We identified RA patients and matched non‐RA comparators among participants consenting to broad research from two large academic centers. We identified incident HF and categorized HF subtypes based on ejection fraction (EF) closest to the HF incident date. Covariates included age, sex, and established CVD risk factors. Cox proportional hazards models were used to estimate the hazard ratios (HR) for incident HF and HF subtypes. Results We studied 1445 RA patients and 4335 matched non‐RA comparators (mean age 51.4 and 51.7 years, 78.7% female). HFpEF was the most common HF subtype in both groups (65% in RA vs. 59% in non‐RA). RA patients had a HR of 1.79 (95% CI: 1.38‐2.32) for incident HF compared to those without RA after adjusting for CVD risk factors. RA patients had a higher rate of HFpEF (HR 1.99, 95% CI: 1.43 ‐ 2.77), but there was no statistical difference in HFrEF rate (HR 1.45, 95% CI 0.81 ‐ 2.60). Conclusion RA was associated with higher rate of HF overall compared to non‐RA, even after adjustment for established CVD risk factors. The elevated risk was driven by HFpEF, supporting a role for inflammation in HFpEF and highlighting potential opportunities to address this excess risk in RA.

Risk of Incident Heart Failure and Heart Failure Subtypes in Patients with Rheumatoid Arthritis

Objective We aimed to characterize hand osteoarthritis (OA) patients with deteriorating or improving hand pain, and to investigate patients achieving good clinical outcome after four years. Methods We used four year annual Australian/Canadian hand osteoarthritis index (AUSCAN) pain subscale (range 0‐20) measurements from the HOSTAS cohort (hand OA patients). Pain changes were categorized as deterioration, stable and improvement using the Minimal Clinical Important Improvement (MCII). Good clinical outcome was categorized using the Patient Acceptable Symptom State (PASS). Associations between baseline characteristics (patient and disease characteristics, coping styles, illness perceptions) and outcomes were investigated using multinomial or binary logistic regression, adjusted for baseline pain, age, sex and BMI. Results 356 patients (83% female, mean age 60.6 years, mean AUSCAN 9.1) were analyzed. Pain improved for 38%, deteriorated for 30% and remained stable for 32% over four years. Four year pain development followed annual trends. At baseline, 44% of patients reached PASS, 49% at follow‐up. Higher BMI, coping through comforting cognitions and illness comprehension were positively associated with pain deterioration. Higher AUSCAN function score, mental wellbeing and illness consequences were negatively associated with pain improvement. Employment (positively) and emotional representations (negatively) were associated with both improvement and deterioration. Higher baseline AUSCAN function, tender joint count and symptoms attributed to hand OA were associated negatively with PASS after four years. Conclusions The pain course of hand OA patients is variable, not inevitably worsening, and various factors may play a role. Whether modification of these risk factors can influence pain outcomes requires further investigation.

Course of hand pain over four years in patients with hand osteoarthritis depends on <scp>BMI</scp>, employment and mental wellbeing: The Hand <scp>OSTeoArthritis</scp> in Secondary care cohort study

Objective Recent research has explored frailty in systemic lupus erythematosus (SLE) using multiple measures. We examined the agreement among frailty measures and the association of each with cross‐sectional and longitudinal health outcomes. Methods We used data from the California Lupus Epidemiology Study (CLUES) to examine the following measures of frailty: Systemic Lupus International Collaborating Clinics (SLICC) Frailty Index (SLICC‐FI), Short Physical Performance Battery (SPPB), and Fatigue, Resistance, Ambulation, Illness, and Loss of Weight (FRAIL) scale questionnaire. PROMIS Physical Function 10a (PF) was tested as a proxy measure of frailty. Agreement between frailty classifications by each measure was assessed. Cross‐sectional associations of frailty classifications with hospitalization, valued life activities disability, cognitive impairment, 6‐minute walk test distance, self‐reported disease damage, fatigue, and depressive symptoms were assessed with logistic and linear regression analyses. Associations with hospitalization, disease damage increase, and disability increase over the subsequent 3 years were assessed Cox proportional hazards analyses. Results Percentages of participants identified as frail varied among the measures, from 10.8% to 45.9%. Agreement among classifications ranged from slight to substantial (κ from 0.17 to 0.63). Most of the frailty measures were associated with both cross‐sectional and longitudinal health outcomes, with the notable exception of the SPPB. SLICC‐FI had the most consistent association with outcomes, followed by FRAIL and PF. Conclusion Multiple measures of frailty appear to identify the risk of poor health outcomes. The intended use, as well as the simplicity and practicality of implementing the measure, may be the most important considerations in choosing a frailty measure.
