Developmental Medicine and Child Neurology

Aim To better understand the neural and performance factors that may underlie developmental coordination disorder ( DCD ), and implications for a multi‐component account. Method A systematic review of the experimental literature published between June 2011 and September 2016 was conducted using a modified PICOS (population, intervention, comparison, outcomes, and study type) framework. A total of 106 studies were included. Results Behavioural data from 91 studies showed a broad cluster of deficits in the anticipatory control of movement, basic processes of motor learning, and cognitive control. Importantly, however, performance issues in DCD were often shown to be moderated by task type and difficulty. As well, we saw new evidence of compensatory processes and strategies in several studies. Neuroimaging data (15 studies, including electroencephalography) showed reduced cortical thickness in the right medial orbitofrontal cortex and altered brain activation patterns across functional networks involving prefrontal, parietal, and cerebellar regions in children with DCD than those in comparison groups. Data from diffusion‐weighted magnetic resonance imaging suggested reduced white matter organization involving sensorimotor structures and altered structural connectivity across the whole brain network. Interpretation Taken together, results support the hypothesis that children with DCD show differences in brain structure and function compared with typically developing children. Behaviourally, these differences may affect anticipatory planning and reduce automatization of movement skill, prompting greater reliance on slower feedback‐based control and compensatory strategies. Implications for future research, theory development, and clinical practice are discussed.

Cognitive and neuroimaging findings in developmental coordination disorder: new insights from a systematic review of recent research

This review outlines a conceptual approach to inform research and practice aimed at supporting children whose lives are complicated by impairment and/or chronic medical conditions, and their families. 'Participation' in meaningful life activities should be an essential intervention goal, to meet the challenges of healthy growth and development, and to provide opportunities to help ensure that young people with impairments reach their full potential across their lifespan. Intervention activities and research can focus on participation as either an independent or dependent variable. The proposed framework and associated hypotheses are applicable to children and young people with a wide variety of conditions, and to their families. In taking a fresh 'non-categorical' perspective to health for children and young people, asking new questions, and exploring issues in innovative ways, we expect to learn lessons and to develop creative solutions that will ultimately benefit children with a wide variety of impairments and challenges, and their families, everywhere.

Participation, both a means and an end: a conceptual analysis of processes and outcomes in childhood disability

Abstract Aim To develop standardized diagnostic criteria for ‘infection‐triggered encephalopathy syndrome (ITES)’ and five specific clinical syndromes of ITES. Method The draft definitions were based on existing criteria, standardized, and discussed by a panel of international experts using nominal group technique over 18 months to achieve consensus. All criteria use the same format: (1) presence of infection/fever; (2) clinical features including encephalopathy; (3) neuroradiological features on magnetic resonance imaging; (4) exclusion of other causes. Results We first highlighted differences between ITES and infectious and autoimmune encephalitis, which is the most important differential diagnosis. Consensus was achieved to define five specific ITESs: acute encephalopathy with biphasic seizures and late reduced diffusion; acute necrotizing encephalopathy; mild encephalopathy with a reversible splenial lesion; acute fulminant cerebral oedema; and acute shock with encephalopathy and multiorgan failure. Two further conditions that are currently classified as epilepsy syndromes but have similar features to ITES, namely febrile infection‐related epilepsy syndrome and hemiconvulsion–hemiplegia–epilepsy syndrome, are also discussed. Interpretation The consensus definition is expected to improve awareness of this disease concept, provide diagnostic framework, and facilitate future international research and clinical trials.

International consensus definitions for infection‐triggered encephalopathy syndromes

There is a growing consensus that the neuropsychological underpinnings of developmental dyscalculia (DD) are a genetically determined disorder of ‘number sense’, a term denoting the ability to represent and manipulate numerical magnitude nonverbally on an internal number line. However, this spatially‐oriented number line develops during elementary school and requires additional cognitive components including working memory and number symbolization (language). Thus, there may be children with familial‐genetic DD with deficits limited to number sense and others with DD and comorbidities such as language delay, dyslexia, or attention‐deficit‐hyperactivity disorder. This duality is supported by epidemiological data indicating that two‐thirds of children with DD have comorbid conditions while one‐third have pure DD. Clinically, they differ according to their profile of arithmetic difficulties. fMRI studies indicate that parietal areas (important for number functions), and frontal regions (dominant for executive working memory and attention functions), are under‐activated in children with DD. A four‐step developmental model that allows prediction of different pathways for DD is presented. The core‐system representation of numerical magnitude (cardinality; step 1) provides the meaning of ‘number’, a precondition to acquiring linguistic (step 2), and Arabic (step 3) number symbols, while a growing working memory enables neuroplastic development of an expanding mental number line during school years (step 4). Therapeutic and educational interventions can be drawn from this model.

Some key stages in the development of manual actions have been discussed in this supplement based on the idea of the dorsal cortical stream as the pathway for translating visual information into action control. We argue that visual information, transmitted through specialized visuomotor dorsal‐stream modules, is required in the control of manual actions for selecting and attending to the target object of the action, translating visual spatial information into motor programmes and planning a coordinated sequence of actions so as to reach an optimal end‐state. In typical development, we illustrate dorsal‐stream processing through results on the use of stereoscopic information to guide infants' reaches, and changes in target selection and detailed kinematics of reaches depending on age, object size, and reaching in darkness (when dorsal‐stream information rapidly decays). We hypothesize ‘dorsal‐stream vulnerability’ as a widespread feature of neurodevelopmental disorders, such as autism, W illiams syndrome, and children born very preterm. Such deficits, identified as abnormal visuomanual actions, are seen in bimanual coordination, visual guidance of action in the ‘postbox’ task, and failures in motor planning for end‐state comfort. We discuss the possible application of these approaches to a wider range of disorders including developmental coordination disorder.

Visual control of manual actions: brain mechanisms in typical development and developmental disorders

Abstract Aim To describe the neurodevelopment and quality of life in SLC13A5 (solute carrier family 13 member 5) citrate transporter disorder (developmental and epileptic encephalopathy 25, DEE25), a rare genetic early infantile epileptic encephalopathy caused by deficiency of a sodium‐citrate transporter, characterized by heavy seizure burden in the neonatal period. Method We analyzed longitudinal neurodevelopmental outcomes from a prospective natural history study of DEE25, using standardized assessments of Mullen Scales of Early Learning, Peabody Developmental Motor Scales, and Vineland Adaptive Behavior Scales. Results There was significant global impairment across the cohort, with variable quality of life and limited genotype–phenotype correlation. Patient‐specific scores were stable across visits with evidence of modest gains in early childhood and static skills in adolescence and adulthood. Interpretation There is a poor prognosis in terms of multiple measures of age‐appropriate development.

Developmental phenotype and quality of life in <scp>SLC13A5</scp> citrate transporter disorder

Abstract Sudden deaths in infants and children represent a profound and tragic event that continues to challenge researchers despite extensive investigation over several decades. The predominant phenotype, sudden infant death syndrome (SIDS), has evolved into the broader category of sudden unexpected infant death (SUID). In older children, a less understood phenomenon known as sudden unexplained death in childhood (SUDC) has garnered attention. Additionally, sudden unexpected death in epilepsy (SUDEP) constitutes a rare but recognized complication of epilepsy. Recent investigations indicate overlapping clinical, neuropathological, and genetic characteristics among SUID, SUDC, and SUDEP. Common features include death occurring during sleep, discovery in the prone position, hippocampal abnormalities, and genetic variations associated with epilepsy or cardiac arrhythmias. Notably, video recordings in certain examples of SUDC have captured ‘convulsive’ episodes preceding death in children without prior seizure history, suggesting that seizures may contribute more significantly to sudden paediatric deaths than previously presumed. This review explores these shared elements, underscoring their importance in formulating possible preventative measures against these devastating conditions.
