Abstract Purpose Normal aging can lead to arterial wall stiffening and development of atherosclerosis; however, the effects of overweight conditions can expedite the dysfunction of arterial health. In an earlier study, we identified in a group of abdominal obese pre-menopausal woman that during menses, these women had decreased vascular conductance of the brachial artery during dynamic hand-grip exercise. Endothelial function in abdominal obese (AO) premenopausal women could be associated with being highly sedentary which attenuates dilatory responses to FMD of both upper and lower limb vascular function. We hypothesized that the AO women would exhibit limb-specific differences in artery dilation with reduced dilation in the popliteal artery as compared to the brachial artery. Methods Artery dilatory responses in the brachial and popliteal arteries were assessed using flow-mediated dilation (FMD) in age-matched sedentary controls and AO groups during menses. Results Significant differences were found between groups including % BF, BMI, weight, waist to hip ratio, fasting blood glucose, and oxLDL. A between-group comparison revealed significantly lower percent dilation of the popliteal artery in AO participants compared to controls (p &lt; 0.05) not present in the brachial arteries. Our results also indicate a difference in the time-to-peak (TP) of dilation between the control (33.38 ± 3.232) and AO (62.104 ± 5.813) groups ( p &lt; 0.01). Following FMD correction with shear rate as the covariate, significant differences in FMD between the brachial and popliteal artery were abolished. Conclusion We conclude that young, AO women show a limb-specific difference in dilation of the upper and lower extremities when compared to controls. Thus, reductions in FMD of the popliteal artery could be due to impaired dilation of the downstream resistance vasculature preventing increases in shear within the leg conduit arteries not as significantly in the arms.

Limb-Specific Comparison of Flow-Mediated Dilation in Abdominal Obese Pre-menopausal Women

Abstract Background Cardiovascular risk factors accelerate vascular ageing beyond chronological age, hence early vascular ageing (EVA). Carotid to femoral pulse wave velocity (cfPWV) is a measure of vascular ageing and is used to identify EVA and supernormal vascular ageing (SUPERNOVA). Vascular ageing is not completely understood in African populations. Therefore, we aimed to phenotype young South African adults stratified by cfPWV extremes in terms of non-modifiable and modifiable risk factors. This study included 1133 young adults (mean age: 24.5 years). We measured cfPWV using applanation tonometry. Body composition measures, self-reported 24-h dietary intake, smoking and alcohol consumption were included. Fasting blood samples were analysed for biochemical risk factors. Three groups based on cfPWV percentiles were compared and included SUPERNOVA (≤ 10th percentile), average vascular ageing (AVA, between 10 to 90th percentile) and EVA (≥ 90th percentile). Results Chronological age, male sex, smoking, alcohol use, and blood pressure were incrementally higher across PWV groups (all p trend ≤ 0.007). Black ethnicity was higher ( p = 0.038) in the SUPERNOVA group. In exploratory factor analysis, a factor pattern including mean arterial pressure and fasting glucose showed beneficial odds (OR 0.62, p = 0.002) for SUPERNOVA and higher likelihood (OR 2.10, p &lt; 0.001) for EVA. Another factor pattern of socio-economic status and total dietary fat showed lower odds (OR 0.64, p = 0.003) for EVA. Conclusion Poor lifestyle behavioural risk factors seem detrimental in the EVA group conferring a possible higher risk of future CVD.

Non-modifiable and Modifiable Risk Factors in Vascular Ageing Extremes: The African-PREDICT Study

Abstract Background Cardiovascular diseases (CVDs) are the most common cause of death worldwide. CVDs share heterogeneous pathophysiologic mechanisms, one of which includes increased oxidative stress. Main Body Surplus levels of reactive oxygen species induce damage to cellular macromolecules such as DNA, proteins, and lipids. Increased reactive oxygen species result in decreased nitric oxide availability, vasoconstriction, and the development of procoagulant and proinflammatory states in blood vessels. Conclusion Improved knowledge of biomolecular processes triggered by oxidative stress has helped develop tools for assessing oxidative stress markers and applying them in clinical settings. Nevertheless, some research gaps should be filled, specifically by defining the most clinically relevant biomarkers for oxidative stress with high sensitivity and specificity for CVD.

Abstract Background The association between cerebrovascular disease and cognitive impairment is well known, but the impact of lower extremity arterial disease (LEAD) on neuropsychological performance is less established. Objectives The aim of this study was to investigate the influence of LEAD on cognitive impairment. Materials and Methods A total of 20 patients with LEAD, classified by Fountain’s stage IIB, qualified for revascularization surgery has been included in this prospective study. Neuropsychological assessments have been done using MoCA and CANTAB test. Fifteen patients qualified for hernia surgery, without peripheral artery disease served as a control group. Linear regression model has been applied to assess the connection between LEAD and cognitive impairment. Results Differences between the study groups reach significance in both MoCA and CANTAB test. In MoCA test, patients with LEAD had lower levels of performance in attention ( p = 0.0254), visuospatial/executive ( p = 0.0343) and delayed recall ( p = 0.0032). The mean MoCA score was below 26 points. In CANTAB test, patients with LEAD performed worse in visual memory and learning. After adjusted for common cerebrovascular risk factors, LEAD was significantly correlated with cognitive impairment defined as MoCA score &lt; 26 points. Conclusions Lower extremity artery disease is associated with cognitive impairment independently of cerebrovascular risk factors.

Abstract Background Antares is an algorithm for oscillometric blood pressure (BP) monitors to determine aortic pulse wave velocity (PWV) solely using oscillometric pulse waves without dependence of any other input. The aim of this study is to test Antares PWV for feasibility and whether the known age and blood pressure dependence of PWV can be shown with Antares PWV. Methods In total, 259 patients were investigated for PWV as sub-study of the invasive validation of Antares algorithm, of which 219 entered analyses. Non-invasive PWV determination by Antares algorithm, integrated into an oscillometric BP monitor (custo screen 400) was compared to five different ePWV equations based on age, BP, or both. Additionally, in a subset of 27 patients, comparison of ARCSolver PWV algorithm (Mobil-O-Graph) with Antares PWV and ePWV was conducted. Results Mean differences ± SD between Antares PWV and (A) ePWV (based on age and systolic BP) was − 0.05 ± 1.06 m/s (Spearman’s rank correlation coefficient, r S = 0.805); (B) ePWV (based on age) was − 1.75 ± 1.17 m/s ( r S = 0.829); (C) ePWV (based on mean BP) was − 1.35 ± 1.24 m/s ( r S = 0.763), and (D) ePWV (based on age and mean BP) was − 1.64 ± 1.22 m/s ( r S = 0.810) and − 1.69 ± 1.18 m/s ( r S = 0.802). Comparison of Antares PWV with ARCSolver PWV revealed a mean difference of − 0.65 ± 1.31 m/s ( r S = 0.854). Conclusion The Antares algorithm confirmed its feasibility to use an oscillometric BP monitor as a single-point measurement device to calculate aortic PWV with acceptable comparability and high correlation to both estimated PWV and ARCSolver PWV. Antares achieves these results solely based on analysis of waveform features without requiring any secondary input, like BP or age.

Abstract Translating vascular aging research from bench to bedside presents both significant opportunities and challenges. This paper summarizes insights from a roundtable discussion at the Artery 23 conference, featuring perspectives from basic science, clinical trials, regulation, and industry. The main conclusions of the discussion are as follows: basic science research must align with clinical relevance, using appropriate animal models and standardized measurement techniques. Pragmatic and registry-based clinical trials offer viable alternatives to traditional randomized controlled trials, facilitating real-world applicability. The regulatory landscape, particularly for software medical devices, must evolve to keep pace with technological advancements like artificial intelligence. Industry efforts focus on developing devices or solutions for vascular aging assessment and treatment strategies, yet face hurdles in large-scale adoption and reimbursement. Despite significant progress, the development of pharmacological interventions to mitigate vascular aging remains a critical need. This discussion underscores the importance of interdisciplinary collaboration to overcome barriers and translate scientific discoveries into clinical practice effectively.

Translational Science in Vascular Aging: From Bench to Bedside—Insights from a VascAgeNet Roundtable

Abstract Background Carotid-femoral pulse wave velocity (c–f PWV) is a gold standard for the arterial stiffness measurement and important biomarker for the assessment of the cardiovascular (CV) risk. Recent studies have focused on 24-h measurements of arterial stiffness and estimated PWV (ePWV). The aim of this study is to analyze agreement of office c–f PWV measurements (SphygmoCor) with 24-h oscillometric measurements (Mobil-O-Graph and Arteriograph), and with ePWV. Results This study included 154 patients with primary hypertension (average age 38.75 ± 12.65). Arterial stiffness has been measured in the office with SphygmoCor and 24 h with two oscillometric methods (Mobil-O-Graph and Arteriograph). ePWV was calculated using validated equation. PWV values obtained in office (SphygmoCor) showed higher average values compared to both 24-h oscillometric measurements of PWV and ePWV. The mean values of 24-h PWV measured by Arteriograph were higher compared to values obtained with Mobil-O-Graph. The measurement of PWV over 24 h using the Arteriograph is the most accurate among the methods that were compared with the office PWV measurements (accuracy of 0.989). However, the most precise method was the Mobil-O-Graph (0.631), and the highest degree of agreement also was shown with the Mobil-O-Graph (concordance coefficient correlation (CCC) = 0.447). The smallest deviation (TDI) and the highest probability of overlapping (CP) were observed with ePWV (TDI = 45.524, CP = 0.322, respectively). Conclusion In our group of young treated mild hypertensive patients with low CV risk, we found weak agreements between cfPWV and 24-h PWV. These methods are not interchangeable.
